Diesel fuel in radiator

My 2002 7.3L Powerstroke (81k) has 1/2 gallon of diesle fuel in the top tank of the radiator. When it get warm the fuel spits out the radiator cap and blows everywhere. No drivability issues, only a strong diesel smell the last couple of weeks. It is at the Dealers now waiting service.
Anyone care to guess what went wrong?
Any previous threads on this problem?.

after initial tests my Ford Dealer doesnt have an answer yet.

still no answer from the dealer despite a visit from a Ford Technition.
After pressure testing he still cant find a leak to the cooling system so he is going to
take out all the injector cups in the head and replace them all.

i have the same problem. Did Ford find the answer? did replacing all of the injector cups help?

I had the same symptons and diagnois this past week. And yes, it was a cracked injector cup/sleeve. i took the truck into the ford dealer. they took off both value covers, removed the injectors, and pressurized the water to determine the location of the leak. cylinder #4 (2nd from front on passenger side) was leaking. in addition to new cup, all injectors were resealed (new orings, washers, and whatever else that means), new heater hoses (9 of them), new radiator hoses, new radiator cap, flush and fill radiator, drain oil and replace filter, and about $2,700 later, the truck is running again at 90,000 miles. i better get another 190k miles before i do this again.

the truck is 2003x, 7.3 L, mostly stock.
